Collecting the results

Bearing in mind safety of your data, we do not give results by phone or to unauthorized persons.

At a request of the client, we send them by priority mail or courier mail. Costs of shipping the results by courier (PLN 25) are borne by the person ordering the test.

If collecting of the result in person is not possible, you should:

• fill in an authorization form, giving name of the person who will collect the result. The indicated person will have to present a document confirming his/her identity and evidence of payment for the test.
  Authorization to collection of the results.
• fill in a form requesting sending of the result by fax. Regardless of the fact of sending the results by fax, the test original will remain in the laboratory for collection.
  Request to send the results by fax.
